A health provider is providing safe care but is under-reporting incidents, according to a watchdog.
The Care Quality Commission inspected Provide, which is based at St Peter's Hospital in Maldon, and offers services across mid Essex.
It said: "In general, we found it provided safe care.
"Most areas had processes in place to recognise and investigate patient safety incidents.
"However, there was an under-reporting of incidents and an absence of shared learning following review of reported incidents."
The CQC said the planning and delivery of services for people who were dying could also be better coordinated.
However, it also added: "The vast majority of people we spoke to were positive about their care but especially in regards to services provided to children and families."
