Maldon: Doctors pilot new way of helping frail patients

GPs are piloting a new system to ensure frail and vulnerable patients are getting the right support to avoid them being admitted to hospital.
Doctors in Maldon and Burnham are using computer software that highlights which patients have the most needs.
Social workers are also linked up with surgeries and once a month will meet GPs, community nurses and social workers to put support packages in place.
Dr Bryan Spencer, vice-chairman of the Mid Essex Clinical Commissioning Group, said: “We are trying to look at services for those patients, mainly because there are very often people that get admitted to acute hospitals that don't need hospital treatment but because they don't manage it it at home when they get a minor illness.”
